This information will be sent to our order department and a Sales Order wil be create and emailed back to you with confirmation of quantites and pricing.

Please review the sales order and confirm its accuracy.

The Sales order will contain a request for deposit - about 40%.
Please pay this deposit to confirm your order.

If you wish to order several different costumes or want costumes for different troupes, complete and send this form once for each troupe.
We will combine troupes on the sales order as needed.

When submitting multiple orders you dont need to repeat your delivery or contact info etc - but please include your studio name and contact name.
